Order Form is a document published in aeronautical publications (AIP/VFR AIP), by which the user requests/renews the ordering service of aeronautical publications.
Order form for subscription and purchase prices are published annually through AIC series A and D and the prices of individual aeronautical charts are published in the subsection GEN 3.2 of AIP/VFR AIP.
